Disability Coverage Options

Carilion offers both short- term and long- term disability coverage. Plan costs are shared between Carilion and our employees.

The Hartford administers our short-term disability, long-term disability, basic life, accidental death and dismemberment and supplemental life insurance plans.

Will I have financial protection if I become disabled?
The short- and long-term disability plans provide financial protection for you and your family by providing a portion of your income if you become disabled. The benefit amount you receive is based on a percentage of covered monthly earnings before your disability. In some cases, you can receive partial disability payments even if you work while you are disabled. Short-term disability does not cover work-related illness or injuries.

What is a disability?
The Hartford’s definition of Total Disability is:

Total Disability means that due to accidental bodily injury, sickness, mental illness, substance abuse or pregnancy you are unable to perform the essential duties of your occupation, and as a result, you are earning less than 20% of your pre-disability Weekly Earnings.

How do I know if my disability is a pre-existing condition?
If your disability begins within the first 12 months of coverage, the carrier will review your claim to determine whether your disability is a pre-existing condition.

When can I sign up for disability benefits?
Short- and long-term disability are optional benefits in which you can enroll when first employed or when first eligible.  If you decline enrollment at that time and wish to enroll later, a Personal Health Application must be completed and submitted to the carrier for underwriting purposes.  Changes in enrollment of disability coverages (addition or deletion) can only be made during the Open Enrollment period.

What steps do I need to take to file a disability claim?

  • Contact your manager if you need to go on a medical leave of absence.
  • Contact your local Human Resources office or the HR Service Center at 983-3600 (33600) or 1-800-599-2537 (toll free) to begin the leave of absence process. They will send you the Carilion Telephonic Brochure or download one here.
  • Sign the card in the brochure and have your physician copy the card for your office file.
  • Call the toll free number to notify the Hartford (1-800-448-5813).
  • The Hartford will contact you if you need to transition to long-term disability and you are enrolled in Carilion's Long-Term Disability program.
