Continuing Medical & Dental Coverage - COBRA

Under provisions of the Consolidated Omnibus Budget Reconciliation Act of 1985, or COBRA, you and your covered family members may choose to continue coverage under Carilion's medical and dental plans after your coverage as an employee is scheduled to end.

Before your last day of employment, confirm that your address is current with Carilion. One way to do this is to see what address is listed on your Carilion paycheck. If that address is incorrect, the address information Carilion has on file for you is incorrect.

You may update your address through My Total Access before you leave Carilion. After leaving Carilion, contact the HR Service Center via e-mail at hrservicecenter@carilion.com or by phone at 983-3600 or 1-800-599-2537 (toll-free) to update your address.

You will receive COBRA information from Infinisource, our COBRA vendor, automatically. This information will reflect your current enrollment in health coverage. Careful planning on your part is required to make COBRA work. You have 60 days from the date your coverage ends to elect continued coverage. If you do not respond within the 60 days, your rights to continue coverage end.

You will pay a monthly premium to continue your coverage. Please note that you may be able to change your coverage from our HMO medical plan to the PPO medical plan if you are moving out of the service area.

Be sure to contact the HR Service Center when you are dropping coverage for a dependent due to a change in circumstances (divorce, ineligibility due to age of a dependent, loss of full-time student status, etc). In some cases this may change your level of coverage (e.g., Family to Employee Plus Spouse). You will need make your elections changes through My Total Access.

Even if you must drop a dependent, or if your coverage terminates due to your death, your dependents may be eligible to continue coverage through COBRA. This may change your level of coverage and you may make changes through My Total Access. If you die, your surviving family members should contact the HR Service Center via e-mail at hrservicecenter@carilion.com or by phone at 983-3600 or 1-800-599-2537 (toll-free) to make changes.

Cost of Continuing Coverage
Human Resources will tell you the exact cost of continuing coverage for you or your family. You will be charged the full cost of the coverage, plus a small administrative fee. (Click here for COBRA Contribution Summary.)

Duration of Your Continued Coverage
The length of time you may continue coverage depends on the reason you or your dependents' coverage ended.

You and/or your dependents may continue coverage for 18 months if you:

  • Change from full time to part-time employment and are working less than 48 hours per pay period.
  • Are laid-off or discharged.
  • Leave work voluntarily for reason other than disability.

You may continue coverage for 29 months if:

  • You leave work because you are totally disabled and eligible to receive Social Security benefits.
  • You or a covered family member become disabled in the first 60 days of COBRA coverage.

Your covered family members may continue their coverage for 36 months if:

  • You die while you are employed by Carilion.
  • Your children reach the dependent age limit for coverage under the Plan.
  • Your spouse or children are no longer eligible for coverage because of separation or divorce.
  • You choose Medicare as your primary carrier.
