BEDFORD MEMORIAL HOSPITAL

Bedford Adult Day Center: Expansion of and enhancements to existing and aging transportation infrastructure to better serve clients utilizing adult day care services.

Bedford Ride: Expansion of a volunteer, non-emergency transportation program for underserved residents.

Central Virginia Community Services: Continuation of successful parenting program that focuses on improving family relationships, parenting skills, and enhancing children’s social and life skills.

CARILION FRANKLIN MEMORIAL HOSPITAL

Family Service of Roanoke Valley — Substance Abuse Program: Implementation of an outpatient mental health and substance abuse counseling services for children and adults.

Free Clinic of Franklin County: Implementation of a hospital discharge medication assistance program, mental health services, and expansion of clinic case management program.

Franklin County Health Department — Adult Dental Program: Oral health program for the provision of dental services to underserved adults.

CARILION GILES MEMORIAL HOSPITAL

Free Clinic of the New River Valley: Program that links underserved clients with healthcare providers for access to primary and preventive care, dental care, lab and diagnostic services, Rx Partnership operations, and medication assistance.

Giles County Public Schools--School Health Program: School-based health access site for the provision of health, education, referral, and medication assistance services and expansion of state insurance enrollment and billing activities.

CARILION ROANOKE COMMUNITY HOSPITAL AND CARILION ROANOKE MEMORIAL HOSPITAL

CHIP of Roanoke Valley: Comprehensive health and case management for underserved families with children though age six; and outreach services to reduce child abuse.

Refugee and Immigration Services — School Liaison Project: School initiative to orient students and parents to U.S. education and health systems, orient and train school personnel, recruit and place tutors and mentors, and facilitate parent-student-teacher interactions.

Carilion Medication Assistance Program: Centralized medication access program serving uninsured clients of Roanoke-based Carilion physicians through the use of pharmaceutical company software that provides free drugs for qualifying patients, especially those with chronic diseases.

Mental Health Association of Roanoke Valley: Pro bono collaborative for the provision of mental health services for uninsured and underserved adults.

Roanoke Valley Academy of Medicine — Project Access: Expansion of project where physician volunteers provide care to low income and uninsured clients in their offices or at the free clinic.

West End Center: Expansion of capacity building activities to sustain after school and out of school youth development, health and tutorial assistance services for at risk children in the inner city.

CARILION NEW RIVER VALLEY MEDICAL CENTER

Free Clinic of the New River Valley — Community Health Access Project: Program that links underserved clients with healthcare providers for access to primary and preventive care, dental care, lab and diagnostic services, Rx Partnership operations and medication assistance.

NRV CARES: Expansion of a parenting program through the merger of Prevent Child Abuse Now and CASA that addresses child abuse prevention and family strengthening through education, advocacy, and community partnerships.

Carilion New River Valley Medical Center — Medication Assistance Program: Expansion of a program to assist uninsured and Medicare clients obtain medications necessary for disease management and therapeutic outcomes.

Phoenix Outreach Partnership — Meals on Wheels: Expansion of a program that utilizes a volunteer network to provide socialization and physician prescribed meals for at risk homebound residents.

CARILION HEALTH SYSTEM

Art Museum of Western Virginia: Capital project to build regional state of the art museum, education center, and dining facilities intended to enhance economic development in the region.

Carilion Breast Care Center — High Risk Genetics Testing Program: Continuation of a program to identify, educate and reduce mortality for those at risk of developing breast and/or ovarian cancer.

Carilion Dental Care—Proud Smiles: Capital expansion and enhancement of existing hospital based pediatric dental program to provide comprehensive oral health services for children and teens.

C2C Home: Implementation of a sustainable home building program intended to improve owner health and quality of life, environment, property values and increase awareness of new methods of home design and construction.

Four Seasons Wellness Center: Construction of a health and wellness center for residents of a rural community in Tazewell County.

Franklin County Library: Completion of a capital project to enhance facilities, expand services, and serve as a catalyst for downtown revitalization.

Franklin County Family YMCA — Youth Program Center: Capital expansion project to provide a youth arts, learning and fitness center.

Mill Mountain Theater: Capacity-building project to implement solid financial, fundraising, governance, and organizational infrastructure to insure the vitality and sustainability of a regional theater.

Planned Parenthood of the Blue Ridge — Reproductive Health Education Program: Continuation of health education programs for health professionals, schools, churches, and youth organizations throughout southwest Virginia.

Pulaski County Humane Society: Completion of a capital project to replace and expand rural animal shelter facility, and implementation of a mandatory spaying/neutering program for all animals leaving the shelter.

Rx Partnership: Expansion of a Commonwealth public-private partnership that solicits free drugs in bulk from participating pharmaceutical companies and arranges for their distribution to licensed pharmacies serving uninsured clients.

Science Museum of Western Virginia — Living River Exhibit and Watershed Gallery: New exhibit  designed to educate about the need to safeguard watersheds and conserve land.

Smyth County Free Clinic: Continuation of a clinic-based medication assistance program serving uninsured, and enhancements to resource development infrastructure.

Southwestern Virginia Second Harvest Food Bank: Capacity building and capital project to implement effective operating infrastructure, enhance existing facility, and secure necessary resources to sustain operations.

The First Tee of Roanoke Valley: Implementation of an inner city program for underserved youth designed to promote character education and development, and life enhancing values through the game of golf.

The Jefferson Center Foundation: Completion of a capital project to enhance facilities, procure furnishings and increase earned income revenues to sustain center activities.

Virginia Atlas of Community Health: Expansion of a comprehensive web-based set of economic, population, and health indicators to assist Commonwealth communities better assess need, necessary interventions and impact.

Virginia Health Care Foundation: Continuation of a collaborative project to fund enhancements in the health safety network and healthcare partnerships throughout southwest Virginia and Commonwealth system initiatives.

Voices for Virginia’s Children: Commonwealth initiative to promote positive changes for kids through the use of Kids Count data books, distribution of policy andadvocacy briefs, and education on and use of self sufficiency standards.

Wellness Center at Meadowbrook: Capital project to renovate an existing building into a wellness center for a rural community.

Western Virginia Land Trust: Collaborative project with the Science Museum of Western Virginia to enhance awareness of need to preserve land viewsheds; work with contractors to improve building and landscape design; and educate public about the value of green space conservation and watershed protection.

Carilion Behavioral Health — Child and Adolescent Psychology Program: Diagnostic and counseling services for children experiencing anxiety, depression, behavioral difficulties, and chronic illness.

Carilion Medical Center for Children — Child Life Program: Psychosocial support services for hospitalized children and their families to help them cope with illness, treatment, and loss of life.

Carilion Patient Transportation Services — Neonatal/Pediatric Transport Team: Regional program to provide transport of medically ill children to Carilion Medical Center for Children for treatment and services.

Children’s Advocacy Center of the Roanoke Valley: Creation of child friendly interview room and viewing room to be used by multi-disciplinary team investigating alleged child abuse cases, training for multi-disciplinary team members, and public awareness campaign.

CHIP of Roanoke Valley: Comprehensive health and case management services for underserved families with children through age six; and outreach efforts to reduce child neglect and abuse.

Roanoke City Prevention Planning Team: Administration of surveys to middle and high school youth attending Roanoke City Public Schools assessing risk behaviors and attitudes, and public awareness campaign on survey results.

Blue Ridge Behavioral Health Care — Mental Health Services for Children Age 0-3: Training program to increase number of mental health professionals and case managers available to diagnose and treat children 0-3 with emotional, behavioral, and developmental problems.

Carilion Behavioral Health — Child and Adolescent Psychiatry Program: Comprehensive outpatient services for children experiencing mental and behavioral disorders.

Carilion Dental Care — Pediatrics: Expansion of hospital-based program to provide comprehensive dental services to underserved children.

Carilion Neonatal Intensive Care Unit — Second Nest Developmental Care Program: Family centered care program to provide education and support, and promote neurological development of pre-term infants.

Carilion Neuro — Developmental Clinic: Multi-disciplinary program to serve children with developmental delays, learning disorders, and behavioral problems.

Carilion Pediatric Clinic — Reach Out and Read: Early literacy program to distribute books to children 6 months to 5 years during well child visits and educate parents on importance of reading aloud to children.

Carilion Pediatric Gastroenterology and Nutrition Clinic — KidFit: Multi-disciplinary, family based program to treat obese children and reduce their development of obesity-related chronic diseases such as Type II Diabetes, Hypertension and Heart Disease.

Roanoke Care Connection for Children — Maxillofacial Clinic: Multi-disciplinary program to serve children with cleft lip and palate disorders.

Roanoke Adolescent Health Partnership: School-based or linked clinics providing access to primary and preventive healthcare services for Roanoke youth.
